Checklist for Staggered Entry
Here are items you will need to bring with you to Staggered Entry (unless you have already turned them in).
1) Blue Health Assessment - This is the physical form from your doctor (usually on blue paper) that lets us know your child's vision, hearing, and other health information. Please make sure the form is filled out completely when you visit your doctor. You will need to make sure you fill out the Parent portion.
2) Up-To-Date Shot Record - This is the list of shots that your child has received since birth. It should be the most current record where all of your child's shots are complete.
According to Wake County, you have 30 days from the first day of school to turn your Blue Health Assessment and Up-To-Date Shot Records in. After those 30 days have passed, your child will be excluded from school until these forms are turned in.
3) Information Sheet - Please click on the link at the bottom, print out the form, fill it out and bring it to school on your child's Staggered Entry day. This form will provide us with lots of information that is necessary for your child's testing day. It needs to be printed on a single page.
4) Information Request - Please click on the link at the bottom, print out the form, fill out the form, and bring it to school on your child's Staggered Entry day.
5) Bring your child at 9:00 to Room #305 on his/her Staggered Entry day with his/her lunch/lunch money, towel, and healthy snack.
6) Be at Root at 3:15 and come to Room #305 to pick your child up after an INCREDIBLE day!
